There is also a Proxy part of this form for patients who are unable to sign or request access themselves, and their representative would need to sign on their behalf.

Patient and representative should attend the surgery together.

Patients will need to print the form off from our website, online portal, bring them to the surgery themselves, sign, provide photographic identification documents such as passport, driving licence and proof of address such as a utility bill, dated within the last 3 months.
